No patient information
The FIND website does not collect protected health information (PHI) or any patient identifiers. The request form asks about your organization and your survey window, not about patients. Please do not put patient information in the message field. If you do, tell us and we will delete it.
What we collect
When you send the request form, we collect what you type into it: your name, title, organization, work email, phone number if you give one, facility type, state, the services you are interested in, your expected survey window, your message and how you prefer to be contacted.
We use this only to reply to your request and to understand who we are speaking with. We do not sell it.
Page views and cookies
We measure page views with Vercel Analytics. It counts visits without setting cookies and without following you across sites, so this site does not show a cookie banner. If that changes, we will update this page and ask first.
Spam protection
To stop automated submissions, we keep your IP address for up to one hour to count how many requests come from it. We do not store it with your request.
Who handles your data
- Supabase stores form submissions in our database.
- Resend delivers the email that tells us about your request.
- Vercel hosts the site and provides page-view analytics.
- Upstash, where enabled, holds the short-lived spam counter.
